a structured session built around you

Nurse reviewing care plan before in-home visit

  • Your nurse reviews your care plan, medications, and health history before arriving.
  • Family members can share concerns or updates in advance so nothing is missed.
  • Supplies, equipment needs, and home environment are confirmed ahead of time.

  • Meals, hydration, and personal care assistance are provided with dignity and respect.
  • Your nurse documents all observations and any changes in condition in real time.

End-of-Session Handover.

  • A written care summary is completed covering everything observed and completed during the session.
  • Family members receive a direct update and can ask questions before the nurse leaves.
  • Recommendations for the next session are outlined to maintain continuity of care.

Consistent in-home nursing care is associated with significantly better recovery outcomes and reduced likelihood of hospital readmission.

Extended care blocks allow your nurse to observe trends over time, enabling earlier intervention and more personalised support.

Patients and Families Who Benefit from Private Care

Our in-home nursing service is designed for individuals who need reliable clinical support at home — whether recovering, managing a long-term condition, or maintaining daily independence safely.

Post-Surgery Recovery

Following major surgery, professional nursing care at home bridges the gap between hospital discharge and full recovery, managing wounds, medications, and mobility with clinical precision.

Chronic Condition Management

Patients living with COPD, heart failure, diabetes, or Parkinson's benefit from regular in-home nursing to monitor, adjust, and prevent condition escalation.

Elderly Living Independently

Frequently Asked Questions

Common questions about our Private Care blocks and extended in-home nursing services.

Each care block includes a registered nurse attending your home for the booked duration. Services during the block may include vital signs monitoring, medication administration, wound care, mobility assistance, personal care, meal support, and documentation. A written care summary is provided at the end of each session.

All Private Care blocks are staffed by registered nurses. Depending on your care needs, a care aide may also be involved under nurse supervision for tasks such as personal care and meal assistance.

The 4-hour block is suited for focused clinical tasks — post-procedure monitoring, morning medication management, or a targeted health check. The 8-hour block covers a full day with all associated care tasks. The 12-hour block is designed for high-dependency care, overnight support, or complex recovery situations where extended nurse presence is essential.

We prioritise continuity of care and will do our best to assign the same registered nurse for recurring sessions. When your primary nurse is unavailable, we ensure a thorough care handover so the incoming nurse is fully briefed.

We support a wide range of conditions including post-surgical recovery, stroke rehabilitation, COPD and respiratory care, diabetes management, Parkinson's disease, dementia and cognitive decline, chronic wound care, and palliative or end-of-life care. Contact us to discuss your specific situation.

Private Care services are not covered by OHIP. Receipts are provided for Health Spending Accounts (HSA) or Flexible Spending Accounts (FSA). Some employer extended health benefits or critical illness plans may include home nursing coverage — please check with your provider.

We recommend booking at least 48 hours in advance to allow us to match the most appropriate nurse to your care needs. For urgent post-hospitalization discharge support, please call us directly and we will do our best to arrange rapid response.

Yes, family members are welcome to be present. Our nurses are experienced at involving families in the care process, answering questions, and providing guidance on supporting your loved one between sessions.

Our registered nurses are trained to assess and respond to changing clinical situations in real time. If a concern arises that requires medical escalation, your nurse will contact the appropriate healthcare provider or emergency services on your behalf.

Yes. We offer expedited discharge planning support. If you or a family member is being discharged from hospital and requires immediate in-home nursing care, please contact us as early as possible so we can prepare a care plan ahead of the discharge date.
