WOXY Care Circle reaches into communities across Greater Toronto, delivering accessible professional nursing care through on-site health screenings, post-discharge support, and caregiver education. Our work is creating measurable community health impact.

Through our work serving communities across Greater Toronto, we consistently observe systemic health management gaps that are especially pronounced among seniors and newcomer populations.
Many seniors lack consistent follow-up after hospital discharge
Blood pressure and diabetes are often unmanaged at the community level
Caregivers are unsure how to carry out medical instructions
Newcomer communities face language barriers in accessing health information
Residents with limited mobility have difficulty attending regular medical appointments
Systematic health data collection is absent at the community level
Undetected risk factors can progress into serious chronic conditions within months without any early intervention.
Post-discharge care gaps result in higher readmission rates and increased burden on families and the healthcare system.
Early screening and community health education are highly effective at preventing costly emergency care and long-term institutional care.
Community-based care models create an essential bridge between primary care and acute hospital services.
WOXY delivers nurse-led community outreach, bringing professional healthcare services directly into communities and providing accessible health support for those who need it most.
